HOW DOES INDUCTION OF LABOR HELP WITH MACROSOMIA?

  43. D2 - S43 · present · positive · Frequently asked Questions and Answers

1- THE PROPOSED INTERVENTION

Labor induction is the use of medications or other methods to bring on (induce) labor. Labor is induced to stimulate contractions of the uterus in an effort to have a vaginal birth. Labor induction may be recommended if the health of the mother or fetus is at risk.

Sometimes labor induction may be needed even if it means that the fetus will be born early. In these cases, the risks of continuing the pregnancy are judged to outweigh the risks of the fetus being born too early.

2- THE REASON (CLINICAL INDICATION) FOR THIS INTERVENTION

MACROSOMIA refers to a baby who is very large at the time of birth. The clinical definition is birth weight over 4000 grams (8 pounds 13 ounces). Macrosomic babies can have difficult and occasionally traumatic births. One approach to try to reduce this trauma has been to induce labor early, so that we may deliver before the baby grows too big.

3- POTENTIAL BENEFITS OF INDUCTION OF LABOR FOR SUSPECTED FETAL MACROSOMIA

In Macrosomic babies, induction of labor before term has been shown to reduce SHOULDER DYSTOCIA, which is when the baby's shoulder becomes stuck during vaginal delivery. There is also reduction in the rate of BONE FRACTURE in the newborn. However, when bone fractures occur, they are usually to the clavicle, which heals well without long-term consequences.

Even though there is the benefit of reduction in shoulder dystocia with induction of labor, there is no proven benefit of reduction to the rate of BRACHIAL PLEXUS INJURY, which is damage to the network of nerves that send signals from the spine to the shoulder, arm and hand of the baby. BRACHIAL PLEXUS INJURY is such a rare event that we can’t draw firm conclusions based on current studies. There is also no proven benefit of reduction in signs of NEONATAL ASPHYXIA, which is when the baby does not get enough oxygen during birth.

4- POTENTIAL RISKS OF INDUCTION OF LABOR FOR SUSPECTED FETAL MACROSOMIA

Estimation of the baby's weight is difficult before birth and is not very accurate. Suspected large babies may tun out to be smaller than expected at delivery, which means parents might be worried unnecessarily and might undergo induction of labor without any benefit.

Induction of labor often takes longer than spontaneous labor at term. It might increase the risk of INSTRUMENTED DELIVERY (vaginal birth assisted by vacuum or forceps) and CESAREAN DELIVERY.

Although some parents and doctors may feel the existing evidence is sufficient to justify inducing labor, others may disagree. Further high-quality studies are needed to find out what is the best time to induce labor towards the end of pregnancy and how to improve the accuracy in diagnosing macrosomia.

5- Frequently asked Questions and Answers are as follows.

WHAT IS MACROSOMIA?
Macrosomia is usually defined as a birthweight above 4000 g, although sometimes a higher cut-off of 4500 g is used (Delpapa 1991; Chatfield 2001: Ju 2009). Large-for-gestational-age fetuses are usually defined as those with a birthweight above the 90th percentile.

HOW DO I KNOW MY BABY IS TOO BIG?
Weighing the infant after the birth is the only accurate way to identify macrosomia. The estimation of the baby's weight is difficult before birth and not very accurate. Clinical estimations of fetal weight are based on feeling the uterus and measuring the height of the fundus of the uterus. Both are subject to considerable variation. HOW DO I KNOW IF I AM AT RISK FOR MACROSOMIA?
Risk factors for giving birth to a large birthweight infant include a previous history of macrosomia, maternal obesity, maternal weight gain during pregnancy, multiparity and male fetus (Berard 1998; Cameron 2014; Gaudet 2014).

WHAT ARE THE RISKS OF MACROSOMIA FOR ME AND MY BABY?
Macrosomia is associated with a prolonged labor and maternal trauma along with a higher risk of birth injuries for the infant and longer-term adverse consequences (Ju 2009; Chatfield 2001; Mohammadbeigi 2013; Perlow 1996). Women with large-for-dates fetuses are more likely to have labor augmentation and caesarean section, more serious perineal trauma, and more severe morbidity. In particular, the risk of shoulder dystocia and the associated risk of neonatal trauma might be reduced by the intervention.
Observational studies cast doubts on the effectiveness of such a policy (Friesen 1995; Weeks 1995) and have suggested that induction increases the risk of caesarean section without reducing birth injury.

HOW DOES LABOR INDUCTION WORK?
Labor induction is the use of medications or other methods to bring on (induce) labor. To prepare for labor and delivery, the cervix begins to soften (ripen), thin out, and open. These changes usually start a few weeks before labor begins. Sometimes when labor is going to be induced, the cervix is not yet "ripe" or soft. This means that labor cannot progress.

With this scoring system, a number ranging from 0 to 13 is given to rate the condition of the cervix. A Bishop score of less than 6 means that your cervix may not be ready for labor.

WHAT IS RIPENING THE CERVIX
Ripening the cervix is a process that helps the cervix soften and thin out in preparation for labor. Medications or devices may be used to soften the cervix so it will stretch (dilate) for labor. Ripening of the cervix can be done with medications or with special devices.

WHAT MEDICATIONS ARE USED TO RIPEN THE CERVIX?
Prostaglandins are medications that can be used to ripen the cervix. They are forms of chemicals made naturally by the body. These medications can be inserted into the vagina or taken by mouth. Some prostaglandins are not used if you have had a previous cesarean birth or other uterine surgery to avoid increasing the possible risk of uterine rupture (tearing).

WHAT DEVICES ARE USED TO RIPEN AND DILATE THE CERVIX?
Laminaria are thin rods inserted into the cervix to dilate it. They are made of a substance that expands when it absorbs water.
A catheter (small tube) with an inflatable balloon on the end. The tube is inserted through the vagina and into the opening of the cervix. Then the balloon expands, which helps open the cervix.

WHAT ARE THE RISKS OF LABOR INDUCTION?
One risk is that when oxytocin is used, the uterus may be overstimulated. This may cause the uterus to contract too often. Too many contractions may lead to changes in the fetal heart rate. If there are problems with the fetal heart rate, oxytocin may be reduced or stopped. Other treatments may be needed to steady the fetal heart rate.
Other risks of labor induction may include:
•	chorioamnionitis, an infection of the amniotic fluid, placenta, or membranes
•	infection of the baby
•	rupture of the uterus (this is rare)
Medical problems that were present before pregnancy or occurred during pregnancy may contribute to these complications. To help prevent these complications, the fetal heart rate and force of contractions may be electronically monitored during labor induction.

WHAT IF LABOR INDUCTION DOES NOT WORK?
Sometimes labor induction doesn't work. If you and your pregnancy are doing well and the amniotic sac has not ruptured, you may be given the option to go home. You can schedule another appointment to try induction again. If your labor starts, you should go back to the hospital.
If you or your baby is not doing well during or after attempting induction, a cesarean birth may be needed. Although most cesarean births are safe, there may be additional risks for you, including:
•	infection
•	hemorrhage (heavy bleeding)
•	complications from anesthesia
The recovery time after a cesarean birth is usually longer than for a vaginal birth.
There are also considerations for future pregnancies. With each cesarean birth, the risk of serious placenta problems in future pregnancies goes up. In addition, the number of cesarean births you have had is a major factor in how you will give birth to any future babies.
